Pleasant Valley, Student Directory Opt-Out

Please fill out the form below ONLY if you wish for your family's information (address and phone) to NOT BE POSTED in the student directory which is sent to all PV families.

Forms must be completed by October 8, 2021.

After that date, the student directory will be pulled together to be published and there will be no ability to opt-out.

Please use the last name as it is displayed in PowerSchool. The names will be used to map to student records and discrepancies between PowerSchools and the name entered may result in an error excluding a family.

Please provide the first name of at least one of the children in the family to uniquely identify the family. Please be sure to match the spelling used in PowerSchool in order to prevent errors in excluding the family.

Please DO NOT include my family's information in the Pleasant Valley School Student directory
